Job summary

Southeastern Integrated Care in Shallotte, NC is seeking an Intake Coordinator to serve as the system navigator and primary contact for clients, families, and referral sources. You will ensure access to immediate and ongoing care, and coordinate communications among care providers, payers, and DSS/HMO partners.

The role requires handling referrals in EMR, coordinating intake within 24 hours, educating clients, and supporting outreach to re-engage clients.

Qualifications

• High school diploma required.
• Familiarity with clinical documentation (CCA; PCP).
• Proficient in Microsoft Office Suite.
• Strong written and verbal communication skills.

Responsibilities

• Capture referrals in EMR accurately and efficiently.
• Engage clients in treatment, educate, counsel, and motivate for recovery.
• Schedule intake within the same day or 24 hours.
• Communicate with referral sources before and after intake.
• Identify referral sources, contacts, and payers involved.
• Coordinate with primary care and SEIC family practice services.
• Review treatment recommendations and relay to intake counselor and Director.
• Send reminder calls/texts prior to intake.
• Maintain registration, including demographic and insurance data.
• Prepare consents and share clinical information with intake counselor.
• Assure 95% of referrals are admitted to treatment.
