Job summary

Enhabit Home Health & Hospice is seeking a coordinator to manage patient referrals and support cross-functional teams in delivering seamless home-based care. The role requires experience in health care, familiarity with Medicare guidelines, and strong customer service skills.

The ideal candidate will have a high school diploma or equivalent, plus experience in home health or pediatrics and proficiency with mobile devices.

Qualifications

• Must possess a high school diploma or equivalent.
• Two years of experience in health care and one year in home health or pediatrics.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
