Enhabit Home Health & Hospice in Colorado is hiring an Intake Coordinator to help coordinate patient referrals and support the transition to services. You will work with branch and sales teams to ensure smooth referrals and adherence to regulatory guidelines.

The role requires strong customer service skills, knowledge of Medicare guidelines for home health or hospice, and the ability to use mobile devices and software. A high school diploma is required, with nursing licensure preferred.

Qualifications

• Must have high school diploma or equivalent.
• At least two years in health care and one year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines for home health or hospice.
• Basic technology skills including use of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
