Job summary

VNS Health is seeking an Intake Specialist to promote collaborative communication with healthcare professionals and coordinate patient admissions. You will obtain and process referrals, confirm demographics and medical information, and support safe transitions of care across care settings.

The role requires strong customer service, knowledge of healthcare terminology, data entry across intake platforms, and the ability to work in a team environment with moderate supervision.

Qualifications

• Bachelor's degree in Health Care, Business or a related field required.
• Minimum of two years' experience in a healthcare business/customer service role.
• Ability to understand medical terminology.
• Experience entering data in a mainframe or similar system.
• Ability to work in a team-based environment with adaptability.

Responsibilities

• Obtains and processes referrals based on admission criteria.
• Collects and confirms demographic and medical information; uses intake platforms.
• Fosters collaboration between VNS Health, facilities, and care team for smooth transitions.
• Determines customer needs and provides accurate intake to ensure satisfaction.
