Job summary

VNS Health is seeking a dedicated Care Coordination Support Specialist to help resolve member-related issues and educate members about plan features and admissions requirements. You will coordinate telephonic intake, respond to non-clinical inquiries, and escalate clinical matters as needed.

The role emphasizes maintaining confidentiality, auditing reports, and supporting care coordination staff while providing multilingual interpretation for members in their preferred language.

Qualifications

• High School Diploma or equivalent required.
• Minimum of one year of customer service experience.
• Language Interpreter: ability to read, write, and speak English and their native or target language.

Responsibilities

• Works with members, providers, and staff to identify and resolve member service and retention related issues.
• Coordinates telephonic intake and responses to clinical and non-clinical customer service issues.
• Assists in monitoring member satisfaction through phone calls and documentation of issues.
