Job summary

Vitra Health in Massachusetts is seeking an experienced Care Access/Intake Manager to lead the Care Access team, oversee the full intake process from referral to enrollment, and ensure continuous authorization for services.

This role drives admissions and revenue, ensures HIPAA compliance, coordinates with Nurse Case Managers and providers, and mentors staff while maintaining compassionate, patient-centered care.

Qualifications

• Associate's degree or equivalent required; Bachelor's degree strongly preferred.
• At least 3 years in healthcare intake, care coordination, or related admin role, with supervision experience.
• Fluency in Spanish, written and spoken preferred.
• Experience with MassHealth, AFC/GAFC, or home care programs, including eligibility verification and PAs.
• Familiarity with CRM/EHR systems such as Salesforce.

Responsibilities

• Lead, supervise, and direct the Care Access team, aligning workflows with MassHealth regulations.
• Oversee full intake process from referral to enrollment and ongoing authorizations.
• Ensure timely follow-up with hospitals, physician offices, and referral sources.
• Maintain documentation integrity and monitor KPIs for admissions timing.
• Coordinate with Nurse Case Managers to schedule assessments and reassessments.
• Maintain HIPAA-compliant handling of protected health information.
• Act as brand ambassador, mentoring team members and managing client communications.
