Job summary

GREEN MEADOWS HOME HEALTH CARE INC is seeking an Intake Coordinator to be the first contact for referrals, patients, and providers. You will manage intake from referral through admission, verify eligibility, ensure documentation completeness, and coordinate with clinical staff to ensure timely admissions while maintaining regulatory compliance.

You will review referrals, verify insurance coverage, obtain orders, and communicate admission timelines to families.

Qualifications

• Minimum of one (1) year of healthcare intake, admissions, scheduling, or customer service experience.
• Knowledge of Medicare, Medicaid, and commercial insurance guidelines preferred.
• Excellent verbal and written communication skills; ability to maintain confidentiality per HIPAA.

Responsibilities

• Receive and process referrals from physicians, hospitals, SNFs, case managers, and other referral sources.
• Review referral documentation for completeness and accuracy.
• Verify patient demographics, insurance coverage, and eligibility for home health services.
• Coordinate with physicians to obtain orders, certifications, and supporting documentation.
• Communicate with patients and families regarding admission scheduling and agency services.
• Enter referral and patient information into the EMR system accurately.
