Job summary

Ascension is seeking a Pharmacist at Ascension St Vincent Indianapolis Hospital in Indianapolis, IN to ensure safe medication use and provide expert pharmaceutical care. The role involves reviewing orders, dispensation, patient and staff education, and collaboration with care teams.

The position requires Indiana-licensed pharmacists, a PharmD or equivalent, and hospital experience or PGY1 residency preferred.

Qualifications

• Indiana license required or obtained within 90 days of hire date.
• Bachelor's or advanced degree in Pharmacy.
• PGY1 residency or hospital pharmacy experience preferred.

Responsibilities

• Review and assess medication orders to identify and prevent drug-related problems; advise formulary alternatives when appropriate.
• Compound and dispense medications for accurate patient delivery.
• Provide drug information and education to medical staff and patients.
• Communicate with physicians and clinicians to facilitate care.
• Provide patient counseling efficiently and effectively when required.

What minimum qualifications you'll need

Licensure / Certification / Registration:

• Pharmacist credentialed from the Indiana Board Of Pharmacy obtained within 3 Months (90 days) of hire date or job transfer date required.

Education:

• Bachelor's or advanced degree in Pharmacy required.

Additional Preferences:

• PGY1 Residency or hospital experience
