Ascension St. Vincent Carmel Hospital in Carmel, IN is hiring a Pharmacist to join the Pharmacy department. This full-time role features rotating shifts, dispensing medications accurately, and providing drug information to the medical staff and patients.

You will review orders, identify drug-related problems, supervise technical personnel, and may precept pharmacy students. Eligibility includes Indiana pharmacist licensure within 90 days, and a PharmD or bachelor's degree in Pharmacy.

Qualifications

• Pharmacist license in Indiana or eligible for Indiana licensure.
• Bachelor's or advanced degree in Pharmacy.
• PGY1 residency and/or prior hospital experience preferred.

Responsabilités

• Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems. Recommends formulary alternatives when appropriate.
• Compounds and dispenses medications for accurate delivery to patients.
• Provides drug information and education to medical staff and patients.
• Communicates with physicians and other clinicians as necessary to facilitate care.
• Oversees technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. Assists with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs.
• Provides patient counseling efficiently and effectively when required.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
• Pharmacist credentialed from the Indiana Board Of Pharmacy obtained within 3 Months (90 days) of hire date or job transfer date.

Education

• Bachelor's or advanced degree in Pharmacy required.
