Job summary

Advocate Health's Good Samaritan Palliative Care team is seeking an inpatient NP to join Monday through Friday coverage. The role requires an active RN and APRN license with appropriate state credentials and national board certification in the field.

The position entails assessment, planning, and coordination of care for age-specific patients within an interdisciplinary team, with emphasis on evidence-based practice and quality outcomes.

Qualifications

• Master’s Degree in Nursing required.
• Active RN and APRN licenses in state of practice.
• Illinois CDS license prior to hire where applicable.

Responsibilities

• Assesses, diagnoses, and determines treatment and management plans for age/acuity; orders and interprets diagnostic studies and prescriptions.
• Manages conditions based on evidence-based care, risks/benefits, and alternatives.
• Provides health promotion and disease prevention counseling for patients and families.
• Works as part of an interdisciplinary team; escalates to physicians when necessary.
• Maintains accurate EMR documentation and submits services per coding and billing standards.
• Facilitates coordinated care and clear communication among healthcare team and agencies.
• Performs procedures per specialty privileges where applicable.
• Participates in quality and safety initiatives and education of staff and students.
