Job summary

Interim HealthCare is seeking a Nurse Practitioner to support our palliative care program. The role is PRN with potential to grow into part-time or full-time, offering meaningful patient-centered care for individuals with complex chronic illnesses.

You will perform comprehensive visits, document encounters in the EMR, collaborate with the interdisciplinary team, and coordinate symptom management and medication plans for medically complex patients.

Qualifications

• Active Nurse Practitioner license in Wisconsin.
• Strong clinical assessment, documentation, and care planning skills.
• Excellent communication and teamwork abilities.
• Experience with eCW EMR is preferred (training available).

Responsibilities

• Conduct comprehensive palliative care visits, including assessment, diagnosis, and symptom management.
• Complete F2F encounters for hospice patients as needed.
• Develop and implement individualized care plans with the interdisciplinary team.
• Prescribe medications and coordinate care for patients with advanced or chronic illnesses.
