Job summary

UnitedHealth Group, part of Elite Palliative Care, is seeking a Palliative Care Nurse to provide direct patient and family care through palliative consultations and to ensure specialty services for seriously ill patients.

Primary responsibilities include applying evidence-based palliative practices, performing comprehensive assessments, coordinating care plans, and maintaining ethical standards. RN/APRN with prescriptive authority and BCLS are required; ACHPN is encouraged after 2 years.

Qualifications

• RN or APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• 5+ years of clinical nursing experience.

Responsibilities

• Demonstrates evidence-based palliative medicine practices in patient care across domains.
• Formulates an effective plan of care using palliative care knowledge.
• Gathers information from records, family, and caregivers for assessment.
• Performs comprehensive assessment and initiates plan with therapeutic interventions.
• ACHPN certification encouraged after 2 years of employment.
