Job summary

The Denver Hospice is seeking a Registered Nurse to join our patient-focused care team in Denver, CO. You will provide skilled nursing support within the Interdisciplinary Team, coordinate care per the Plan of Care, and educate patients and families about hospice options.

Qualified candidates are Colorado-licensed RNs with 1–2 years in hospice, palliative, or acute care settings, and CPR certification within 14 days of hire.

Qualifications

• Graduate of an accredited school of nursing required.
• Minimum 1–2 years of hospice/palliative care or acute care experience preferred.
• Current Colorado RN license and valid driver’s license required. CPR certification required within 14 days of hire.

Responsibilities

• Provides skilled nursing coordination of care to patients (neonates to geriatrics) per the Plan of Care.
• Collaborates across disciplines to coordinate patient care and participate in IDT/IDG meetings.
• Educates patients, families and caregivers on hospice philosophy and care options.
• Supervises CNAs and delegates tasks per policy.
• Contributes to community education, staff training and orientation efforts.
• Manages outpatient and inpatient nursing needs as part of the care team.
