Job summary

The Denver Hospice is seeking a Nurse Practitioner to join our Palliative team. This full-time role follows a Monday–Friday, 8–5 schedule with rotating on-call in Denver, Colorado.

You will evaluate and treat patients in hospice, palliative, and home health settings as part of the interdisciplinary team, manage pain and symptoms, write orders, and coordinate with physicians and nurses. Requires CO RN/APN licenses, board-certified NP, DEA, and CPR within 14 days.

Qualifications

• One to two years of Nurse Practitioner experience; hospice, PACE or related field preferred.
• Current Colorado RN and APN/NP licenses; DEA number and prescriptive authority required.
• Board certified as Adult or Family NP by AANP or ANCC.
• Valid Colorado driver’s license.
• CPR certification required or to be obtained within 14 days of hire.

Responsibilities

• Evaluate and treat patients in hospice, palliative, and home health programs as a member of the IDT/IDG.
• Perform exams, assess health status, and determine care plans and differential diagnoses.
• Determine admissions/transfers, provide care recommendations, and coordinate with providers.
• Write orders and prescribe medications for symptom control; communicate with staff and referral sources.
• Order and interpret labs/tests, conduct recertifications, maintain accurate records.
• Support clinical staff and ensure proper medical documentation with HIM.
