Job summary

Optimal Hospice is seeking a Registered Nurse Case Manager to provide comprehensive hospice care for patients and families in Colorado. You will perform initial and ongoing assessments, develop and implement care plans, coordinate with the interdisciplinary team, and educate families to support comfort and quality of life.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating needs, and revising plan as necessary.
• Initiates appropriate preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Teaches the patient and family self-care techniques and coordinates care with the Interdisciplinary Group.
