Job summary

Sentara Norfolk General Hospital in Norfolk, VA is hiring an Inpatient Case Manager/Social Worker (MSW). This full-time role offers weekend-day schedules with 3×12 or 4×10 options and a $15,000 sign-on for qualified candidates.

You will assess social, cultural, and financial factors, collaborate with the healthcare team to develop transition plans, and coordinate community resources to support patients during hospitalization.

Qualifications

• MSW is required (Master’s Level Degree in Social Work).
• BLS certification within 90 days of hire.
• Case Management certification within one year of eligibility (ACM/CCM/CCCTM/CMAC/CMGT-BC).
• 1 year of acute care/healthcare experience preferred (internship accepted).

Responsibilities

• Provide medical social work services to in-hospital patients.
• Assess social, cultural, environmental, and financial needs of referred patients.
• Collaborate with healthcare team and community resources to address needs.
• Develop effective transition plans in consultation with the healthcare team.
• Coordinate with RN Case Manager to ensure smooth patient flow during stay.
• Practice in Hospital settings as outlined (Hospitals Only).
