Qualifications

• Master degree in Social Work (MSW).
• 1 year experience in a relevant setting.
• Certification in Case Management within one year of eligibility.
• BLS certification within 90 days of hire.

Responsibilities

• Provide medical social work services to hospital patients and assess social, psychological, cultural, environmental, and financial needs.
• Collaborate with patients, families, healthcare team, and community resources to identify needs and arrange appropriate services.
• Develop and evaluate transition plans considering diagnosis, prognosis, and benefits across the continuum of care.
