Job summary

Sentara Princess Anne Hospital in Virginia Beach is hiring an Inpatient Case Manager/Social Worker (MSW) to provide medical social work services within the hospital and collaborate with the care team.

This full-time onsite role involves assessing patient needs, coordinating with the healthcare team, developing discharge plans, and ensuring smooth patient flow with the RN Case Manager. A 15k sign-on is offered for qualified candidates, with a Monday–Friday schedule and rotating weekend coverage.

Qualifications

• MSW required.
• BLS certification required within 90 days of hire.
• Case Management certification (ACM/CCM/CCCTM/CMAC/CMGT-BC) required within one year of eligibility.
• 1 year of acute care and/or healthcare experience required (internship acceptable).

Responsibilities

• Provide medical social work services to hospital patients.
• Assess social, cultural, environmental, and financial needs of patients.
• Collaborate with healthcare team and community resources to identify needs and arrange resources.
• Develop effective discharge and transition plans in consultation with the team.
• Coordinate with the RN Case Manager to ensure smooth patient flow through hospital stay.
• Practice in Hospitals Only setting.
