Job summary

Methodist Hospital Northeast is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions for inpatient floors and the outpatient ED. The MSW will assist with discharge planning, crisis intervention, and referrals, coordinating with care teams to address financial, emotional and psychosocial needs.

Qualifications

• Master's degree in Social Work from an accredited college or university.
• Two years hospital experience preferred.
• Social Work license (LMSW) required.
• Bilingual preferred, but not required.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance companies and staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual aspects.
• Maintain knowledge of Medicare, Medicaid and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings with care teams, patients, families and caregivers.
• Provide psychosocial support, counseling, advocacy and referrals for emotional and financial needs.
• Offer emotional support to patients and families during end-of-life, crisis or terminal illness situations.
