Job summary

Methodist Health System is seeking a Social Worker to provide direct and indirect social work services to patients and families. The role includes psychosocial assessment, discharge planning, and coordination with a multidisciplinary team to support treatment and discharge plans, with education and documentation responsibilities.

Requires a Master’s in Social Work and Texas LMSW/LSW eligibility, plus 1 year in a health-related setting.

Qualifications

• Master's Degree in Social Work required.
• 1 year of experience in health-related setting preferred.
• LMSW/LSW or eligibility for licensure in Texas required within one year of hire.

Responsibilities

• Assist with patient assistance programs as directed.
• Consult with physician and multidisciplinary team to address psychosocial and environmental factors affecting treatment or discharge.
• Coordinate resources and maintain knowledge of Federal, State and community resources.
• Oversee social work interns and volunteers.
• Participate in educational activities, care conferences, and staff meetings.
• Perform records in designated software and maintain all documentation including psychosocial assessments.
• Plan for discharges and promote interdisciplinary care planning.
