Job summary

Research Medical Center in Kansas City, MO seeks a compassionate Social Worker (MSW) to support inpatient and ED patients with biopsychosocial assessments and discharge planning. This role collaborates with care teams to identify and address patient needs and connects families with resources to facilitate safe transitions.

You will work to remove discharge barriers, navigate Medicare/Medicaid and private payer regulations, and coordinate meetings with families to support care decisions.

Qualifications

• Masters Degree in Social Work or related field.
• Minimum of one (1) year of hospital Social Work experience is preferred.

Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
• Identify and address barriers to discharge including financial, emotional, psychosocial, medical, and spiritual aspects.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers; assist in difficult decisions while honoring preferences.
• Provide emotional support to patients and family members during complex and emotionally charged issues, including end-of-life and crisis interventions.
