MedStar Health is seeking an Inpatient Case Manager at MedStar Union Memorial Hospital in Baltimore. This role focuses on coordinating care from pre-admission through post-discharge, collaborating with multidisciplinary teams to optimize outcomes.

The ideal candidate has a CSWE-accredited MSW and 1–2 years in hospital social work or case management, with licensure in LCSW-C or LICSW and CCM preferred.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• 1–2 years' experience in social work, preferably in a hospital setting.
• 1–2 years' experience in care/case management.
• Licensure: LCSW-C or LICSW depending on location.
• CCM certification preferred.

Responsibilities

• Develop and coordinate plan of care across the continuum with defined goals and timeframes.
• Collaborate with community agencies for referrals and resources.
• Facilitate communication among patient, family, and healthcare team; coordinate discharge planning.
• Conduct psychosocial history or socioeconomic assessment as required.
• Maintain information on community resources and referral requirements.
