Job summary

MedStar Health’s Washington Hospital Center in Baltimore, MD seeks an LMSW/LMGW Inpatient Case Manager to join our Case Management Team. You will facilitate quality, cost-effective, patient-centered care from pre-admission through post-discharge and help meet individualized outcomes.

The role requires strong communication, collaboration with the care team, and experience in hospital case management. CCM preferred; license in DC or MD is required based on work location.

Qualifications

• Master's degree in social work from an accredited school.
• Valid Social Worker license in DC or Maryland depending on work location.
• Experience in hospital social work or case management preferred.
• CCM - Certified Case Manager preferred.

Responsibilities

• Collaborates with multidisciplinary team to develop and coordinate the plan of care with short- and long-term goals and involved family/caregiver.
• Communicates with caregivers and case management triad on discharge planning and payer concerns.
• Maintains timely documentation and coordinates forms for services required.
• Facilitates discharge planning rounds and coordinates with internal and external providers.
• Manages case load and prioritizes tasks within the patient’s medical plan.
