Qualifications

• 1–2 years of experience in hospital social work (preferred).
• 1–2 years of experience in care/case (preferred).
• Master’s of Social Work (MSW) degree required.

Responsibilities

• Provides social work and discharge planning services including assessment, planning, and intervention to patients and families.
• Identifies patients requiring social work intervention through established case finding mechanisms; conducts biopsychosocial assessments.
• Develops discharge plans responsive to patient/family concerns and coordinates with care teams.
• Coordinates activities with hospital staff and referral agencies to facilitate patient care and post-discharge needs.
• Documents social work assessment, plans, and outcomes in the medical record; completes required reports.
• Mentors and trains staff and students; participates in quality improvement projects and inter-departmental initiatives.
