Job summary

MedStar Washington Hospital Center is actively seeking a Social Work Team Lead Inpatient Case Manager to drive discharge planning and coordinate patient care. The role collaborates with physicians, nurses, CRM Case Managers, and outside agencies to ensure timely post-acute services and comprehensive discharge plans.

The position requires an MSW from a CSWE-accredited program and a DC LICSW license, with 1–2 years of medical setting experience preferred.

Qualifications

• DC LICSW licensure required.
• 1–2 years of medical setting experience preferred.

Responsibilities

• Interviews patients/families to assess needs and plans interventions; documents progress and coordinates discharge planning.
• Provides counseling to patients and families adjusting to illness and treatment; supports transition management.
• Identifies barriers to discharge and coordinates resources with internal/external care providers.
• Maintains clinical documentation in Allscripts and other systems; provides weekly supervision to 2–3 colleagues seeking licensure.
• Prepares and disseminates legal and clinical documents; may lead high-profile cases when needed.
