Job summary

MedStar Health is seeking an LMSW/LMGW Inpatient Case Manager to join our Case Management Team in the Baltimore area. The role focuses on coordinating plans of care, discharge planning, and ensuring patient-centered outcomes during acute hospitalization.

You will collaborate with a multidisciplinary team, document activities thoroughly, and facilitate referrals to community resources to support transition from hospital to home or other care settings. CCM certification is preferred.

Qualifications

• Master's degree in social work from a school accredited by the Council on Social Work.
• Valid Social Worker license in the District of Columbia or Maryland depending on work location.
• Experience in social work in a hospital setting preferred.
• Experience in care/case management preferred.
• CCM - Certified Case Manager preferred.

Responsibilities

• Collaborates with the multidisciplinary health care team to develop and coordinate the plan of care, including long- and short-term goals with timelines and family involvement.
• Communicates with direct caregivers and case management triad regarding discharge planning, LOS, and payor concerns; initiates timely referrals.
• Maintains accurate and timely documentation of case management activities and coordinates discharge plans with physicians, caregivers, and families.
• Develops and maintains information on community resources; facilitates transitions and referrals based on patient population needs.
• Organizes workloads, provides clinical assessment and disposition for behavioral health concerns or illness.

Education
