Qualifications

• Collaborates with the multidisciplinary healthcare team to assess patient needs and coordinate individualized plans of care.
• Performs psychosocial and socioeconomic assessments and connects patients with community resources.
• Communicates with patients, families, and providers to address social, emotional, cultural, and financial barriers to care.
• Documents assessments, referrals, and interventions and ensures compliance with policies and standards.

Responsibilities

• Collaborates with the multidisciplinary healthcare team to assess patient needs, develop and coordinate individualized plans of care, and facilitate safe, timely discharges.
• Conducts psychosocial and socioeconomic assessments, provides clinical evaluations for behavioral health concerns, and connects patients with appropriate community and social service resources.
• Facilitates communication between patients, families, and healthcare providers to address social, emotional, cultural, and financial barriers to care.
• Documents assessments, referrals, and interventions accurately while maintaining compliance with departmental policies, quality standards, and regulatory requirements.
• Participates in quality improvement initiatives, meetings, and professional development to enhance care management effectiveness and departmental performance.
