NewYork-Presbyterian Allen Hospital is seeking a Care Manager registered nurse to join our inpatient units on the day shift. You will lead intensive case management for adults with complex medical needs and coordinate with an interdisciplinary team to optimize length of stay and prevent readmissions.

The role requires a BSN, New York State RN licensure, and 3–5 years of acute care experience. Preferred qualifications include case management experience, a master's degree, and Spanish language

Formación

• BSN required.
• Active NYS RN licensure.
• 3–5 years of acute care experience.

Responsabilidades

• Provide intensive case management for adult inpatients with complex medical needs.
• Serve as liaison between interdisciplinary team (social workers, physicians, therapists) and patients/families.
• Develop individualized care plans and manage length of stay; help prevent readmission and coordinate post-discharge services.
