Job summary

Qualifications

• Bachelor's degree in Nursing with NYS RN licensure.

Responsibilities

• Provide intensive care management for adult patients with complex medical needs.
• Serve as liaison between interdisciplinary team (social workers, physicians, therapists).
• Develop individualized care plans and coordinate inpatient care to ensure optimal length of stay.
• Help prevent hospital readmission and plan outpatient services prior to discharge.
• Lead continuity of care from pre-admission to discharge to ensure quality outcomes.
