Responsibilities

• Identifies cases that require peer review in accordance with clinical indicators and criteria developed by the clinical department.
• Applies customary protocols, pathways, evidence-based processes and other means of managing patient care.
• Supports the mission, philosophy, standards, goals and objectives of NYU Hospitals Center and Care Management Program.
• Communicates outcome of chart review and managed care reviews with the health care team; performs and documents initial certification and continued stay reviews; maintains payor authorization communications.
• Acts as advocate for insurance-related issues and collaborates to maintain appropriate levels of care.
• Participates in departmental and hospital committees; presents updates to ensure continuous improvement.
• Assesses documentation for appropriate admission level of care and facilitates discharge planning planning; communicates with physicians and teams.
• Documents assessments, findings and outcomes in care management systems; contributes to quality improvement strategies.
• Develops strategies to address transitional planning needs and measures improvements in care management delivery.
• Uses evidence-based practice to drive improvement strategies and facilitates rounds to identify patient care needs and discharge planning dates.
• Coordinates care across the continuum and ensures timely movement through levels of care.
