Job summary

NewYork-Presbyterian seeks a Care Manager - RN for the Medicine Units to manage complex cases and ensure continuity of care across the patient journey in a leading academic medical center. BSN is required; 3–5 years in acute care with 2 years in case management or care coordination are preferred, plus NYS RN licensure.

You will lead multidisciplinary teams and oversee inpatient care and discharge planning to optimize length of stay.

Qualifications

• BSN required.
• 3–5 years of healthcare experience in an acute care setting.
• 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting preferred.

Responsibilities

• Manage complex cases and serve as a liaison between acute care and critical care teams.
• Coordinate efforts of social workers, feeding specialists, PTs, OTs, cardiologists, intensivists, and neonatologists.
• Direct all aspects of inpatient care, ensuring optimal length of stay.
• Anticipate outpatient service needs, setting appointments and consultations prior to discharge.
