Job summary

NewYork-Presbyterian Hospital/Weill Cornell Ambulatory Care Network seeks an RN Care Manager for Pediatric Complex Care Management. You will develop care plans, coordinate with a multidisciplinary team, and focus on reducing readmissions across the ACN.

The ideal candidate holds a BSN, NYS RN licensure, and 5+ years in health care; bilingual English/Spanish is preferred. Role is full-time, hybrid, Mon-Fri 9:00am-5PM; on-site presence required.

Qualifications

• Two years case management in inpatient, HMO, Insurance or ambulatory setting (from preferred criteria).
• BSN required; RN license in NYS.
• Five+ years of healthcare experience.

Responsibilities

• Provide intensive care management for pediatric patients with complex medical needs.
• Develop individualized care plans and coordinate with physicians, social workers and others.
• Help minimize hospital readmissions and guide families through care navigation.
