NewYork-Presbyterian Allen Hospital in New York, NY seeks a Care Manager - RN for Inpatient Units on Day Shift. You will lead intensive case management for adults with complex medical needs, serving as liaison among social workers, physicians, therapists, and families.

This full-time day position emphasizes autonomy, quality outcomes, and discharge planning to optimize length of stay and prevent readmission. Must hold BSN and NYS RN license; Spanish fluency helpful.

Competenze

• 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting.
• Master's Degree in healthcare related field.
• Fluency in Spanish.
• Bachelor's degree in Nursing (BSN) and NYS RN licensure are required.
