Job summary

NewYork-Presbyterian hospital in New York, NY seeks a Care Manager (RN) for inpatient units at Columbia Day Shift. You will lead care coordination for adults with complex medical needs, collaborating with physicians, social workers, and therapists to craft individualized care plans and ensure smooth discharge planning.

Strong leadership and communication skills are essential. Spanish fluency preferred, BSN and NYS RN licensure required, with the expectation of 2–5 years in acute care settings

Qualifications

• Two years of case management/care coordination experience in inpatient, HMO, insurance, or other outpatient setting
• One year of leadership experience in a healthcare setting
• Master's Degree in healthcare-related field required
• Bachelor's degree in Nursing (BSN) and current NYS RN licensure required
• 3 to 5 years of healthcare experience in an acute care setting
• Superior communication, problem solving, business acumen, and interpersonal skills are critical

Responsibilities

• Provide intensive care management for adult patients with complex medical needs
• Serve as liaison between interdisciplinary team (social workers, physicians, therapists) and families
• Develop individualized care plans and ensure optimal length of stay and prevent readmission
• Coordinate discharge planning including appointments and consultations
• Lead nursing practice improvements within inpatient units
