Job summary

Advocate Health seeks a part-time RN Case Manager in Charlotte, NC to conduct comprehensive assessments, develop care plans, and coordinate discharge planning. You will collaborate with physicians, social work, and ancillary services to ensure timely, safe transitions and regulatory compliance.

The role requires at least 3 years of nursing experience, preferably with case management, and an RN license (BSN preferred).

Qualifications

• Typically requires nursing experience with case management or discharge planning.
• Familiarity with CMS regulations and utilization management.
• Ability to coordinate care across multiple disciplines and services.
• Strong critical thinking and communication skills.

Responsibilities

• Conducts complete assessments and develops care plans for discharge.
• Coordinates timely transitions and communicates with patient, family, and care team.
• Performs case management across varying levels of care and payer requirements.
• Initiates referrals and documents UM activities per standards.
• Educates team on admission status, care coordination, and regulatory expectations.
• Maintains knowledge of regulatory requirements and CMS guidelines.
