Job summary

Atrium Health University City Case Management is seeking a PRN Care Manager with an RN license and case management experience. Open shifts include Monday-Friday 0800-1630 and Saturday/Sunday 0700-1900, with part-time status and variable hours.

Responsibilities include utilizing the RN process, coordinating care across settings, maintaining case records, and ensuring reimbursement documentation. Strong communication and leadership are essential for success in this role.

Qualifications

• BSN required; Masters preferred; Current RN license in state of work.
• Two years' healthcare experience; case management/discharge planning in Acute care, Home care, LTC, Physician Office, or Managed Care.
• Professional certification required within 3 years of hire date.
• Clinical competence in disease management and case management principles.
• Excellent interpersonal communication, negotiation, problem-solving, organizational and time management skills; ability to work independently.

Responsibilities

• Utilizes RN process to guide care team toward optimal outcomes, resource utilization, clinical expertise, and improvement strategies.
• Interacts with patients, professionals, and the community to coordinate care and document plans across settings.
• Performs comprehensive needs assessment and matches services to identified needs; manages disease management for health maintenance.
• Develops and maintains accurate case records; documents fully and uses appropriate databases and documentation systems.
• Maintains knowledge of reimbursement criteria for Medicaid, Medicare, and Managed Care; ensures proper documentation.
• Demonstrates leadership in professional nursing practice and adheres to standards, rules, and regulations.
