Job summary

Atrium Health Cleveland is seeking a part-time Case Manager in our case management department. On-site coverage is required at Atrium Health Cleveland with weekday hours and additional weekend coverage as scheduled.

The role involves coordinating care across the care team, managing the caseload, and leveraging resources to resolve barriers to care while maintaining regulatory compliance. RN licensure and a nursing degree are required.

Qualifications

• Registered Nurse license in the state where you practice.
• Bachelor's Degree in Nursing or related field.
• Typically requires 5 years of experience in clinical nursing or 1-2 years of care management experience.

Responsibilities

• Facilitates communications among patient/family, multidisciplinary team, medical management team, community resources and other disciplines to anticipate, identify, evaluate, and act to resolve barriers to timely care delivery.
• Collaborates with patient/family and inter-professional teams to provide a model of care that ensures quality, efficient, and cost-effective services.
• Uses evidence-based approaches to increase patient and family activation and engagement in their own care and develops SMART goals.
• Identifies barriers to learning or optimal care delivery and reports findings to the care team with a plan.
• Independently manages CM caseload according to department expectations and ensures timely documentation for regulatory requirements.
• Partners with at-risk patients through diagnosis, treatment and follow-up to deliver continuity of care.
• Coordinates patient information between patient/family, facilities, physicians, and community caregivers to ensure smooth transitions of care.
• Coordinates referrals to internal departments and external community resources as needed.
• Demonstrates knowledge to provide age-appropriate care across the life span.
