Job summary

Atlantic Health System is seeking an Inpatient Care Manager (RN) - Per-Diem for Days (8 AM-4 PM) at the Atlantic Health Newton/Hackettstown Medical Center in Newton, NJ. This role requires coordinating patient care across the continuum and leading interdisciplinary efforts to ensure quality outcomes.

Qualified candidates will hold an ASN, a current NJ RN license, and preferably BSN with 3+ years of acute care experience.

Qualifications

• ASN required; Current, valid New Jersey RN license is required.
• BSN degree preferred.
• Minimum of three years of acute care experience strongly preferred.

Responsibilities

• Manage, coordinate, and facilitate patient care through assessments and multidisciplinary collaboration.
• Perform daily chart reviews and coordinate discharge planning with thorough clinical documentation.
• Lead interdisciplinary rounds and address barriers to care progression and discharge delays.
• Develop and implement individualized care and discharge plans for safe and timely transitions.
• Collaborate with the care team to support patient care plans.
• Track outcomes and analyze pathways to identify improvement opportunities.
• Represent the department in hospital and community activities.
