Job summary

Atlantic Health Chilton Medical Center in Pompton Plains, NJ is seeking an Inpatient Care Manager (RN) for a per-diem day shift from 8 AM to 4 PM. You will oversee care across the continuum, lead an interdisciplinary team, and collaborate with case management to optimize outcomes and resource use.

Required is a Bachelor's degree in Nursing and a valid New Jersey nursing license; a BSN is preferred and 3+ years of Acute Care experience are strongly preferred.

Qualifications

• Bachelor's degree in Nursing.
• Valid New Jersey Nursing License Required.
• BSN preferred.
• Minimum of 3 years of Acute Care experience strongly preferred.

Responsibilities

• Manages, coordinates, and facilitates the care delivered to patients through patient/family assessment and multidisciplinary collaboration.
• Ensures daily chart review, utilization reviews, coordination of discharge planning activities and comprehensive clinical documentation in the electronic medical record.
• Provides interdisciplinary patient care rounds, and identifies and works to resolve quality of care issues, barriers to the progression of care and discharge delays.
• Formulates, coordinates and implements discharge plans and an efficient plan of care to ensure safe, timely and effective discharge plan.
• Works with the interdisciplinary healthcare team to manage and assist the patient care plan.
• Performs variance tracking, outcome analysis and identifies problems to assess the effectiveness of the clinical pathways.
• Servers as a representative of the department and for the hospital community outreach.
• Performs other related duties as assigned.
