Qualifications

• ASN required with current NJ RN license.
• BSN preferred.
• Minimum three years acute care experience strongly preferred.

Responsibilities

• Coordinate and facilitate patient care through assessments and multidisciplinary collaboration.
• Perform daily chart reviews and utilization reviews; coordinate discharge planning and documentation in EMR.
• Lead interdisciplinary rounds; identify and resolve barriers to care and discharge delays.
• Develop individualized plans of care and discharge plans for safe transitions.
• Collaborate with health care team to support patient care plans.
• Track outcomes to identify opportunities for improvement.
