You will manage care for selected patient populations, lead interdisciplinary rounds, ensure effective utilization, and coordinate discharge planning to optimize patient outcomes.

Requires NJ RN license and a Bachelor's degree in Nursing, with preferred 3+ years in acute care. The role emphasizes chart reviews, documentation in the EMR, and collaboration with the care team

Qualifications

• Bachelor's degree in Nursing and active NJ RN license.
• Experience in inpatient acute care setting preferred.

Responsibilities

• Coordinate patient care across the interdisciplinary team.
• Conduct daily chart reviews and utilization reviews.
• Lead discharge planning activities and document care in the EMR.
• Participate in interdisciplinary patient rounds to address care barriers and delays.
• Develop and implement efficient, safe discharge plans.
