Job summary

Atlantic Health System is seeking an Inpatient Care Manager (RN) for Per-Diem days (8 AM - 4 PM) at Chilton Medical Center. The role focuses on coordinating care across the continuum, managing discharge planning, and leading interdisciplinary rounds in Pompton Plains, NJ.

Qualifications include a Bachelor's degree in Nursing and a NJ nursing license; BSN preferred and 3+ years of acute care experience strongly preferred.

Qualifications

• Bachelor's degree in Nursing.
• Valid New Jersey Nursing License Required.
• BSN preferred.
• Minimum of 3 years of Acute Care experience is strongly preferred.

Responsibilities

• Manages, coordinates, and facilitates the care delivered to patients through patient/family assessment and multidisciplinary collaboration.
• Ensures daily chart review, utilization reviews, coordination of discharge planning activities and comprehensive clinical documentation in the electronic medical record.
• Provides interdisciplinary patient care rounds, and identifies and works to resolve quality of care issues, barriers to the progression of care and discharge delays.
• Formulates, coordinates and implements discharge plans and an efficient plan of care to ensure safe, timely and effective discharge plan.
• Works with the interdisciplinary healthcare team to manage and assist the patient care plan.
• Performs variance tracking, outcome analysis and identifies problems to assess the effectiveness of the clinical pathways.
• Servers as a representative of the department and for the hospital community outreach.
• Performs other related duties as assigned.
