Job summary

Atlantic Health seeks an Inpatient Care Manager (RN) for Per-Diem days (8 AM - 4 PM) at Overlook Medical Center, Summit, NJ. You will lead care coordination across the continuum, ensure appropriate utilization, and guide an interdisciplinary team to optimize patient outcomes.

Responsibilities include chart reviews, discharge planning, clinical documentation, and identifying barriers to timely care. Must hold a NJ nursing license and a BSN; acute care experience is preferred.

Qualifications

• Bachelor's degree in Nursing or related field (required).
• Valid New Jersey Nursing License (required).
• Minimum of 3 years of Acute Care experience (strongly preferred).

Responsibilities

• Manages care across the continuum of illness and coordinates multidisciplinary collaboration.
• Performs daily chart review, utilization reviews, discharge planning, and clinical documentation in the EMR.
• Conducts interdisciplinary rounds and addresses barriers to progression of care and discharge delays.
• Develops and coordinates discharge plans to ensure safe, timely, and effective patient discharge.
• Represents the department and engages in hospital community outreach.
