Job summary

Option Care Health is seeking a Registered Nurse II to provide intermediate direct patient care in patients' homes or alternate infusion suites. The role supports continuity of care under a physician’s plan, with supervision.

Shifts are 40 hours weekly, typically Tue–Sat, with travel to in-home and in-suite infusion locations. The position requires an active RN license, a minimum of 1 year of nursing experience, and CPR certification.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments; administers medications and educates patients and families.
• Assesses patient needs and status at each visit; clarifies orders and reports significant changes to physicians.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Communicates with clinical teams, patients, and families to coordinate care and complete timely documentation.
• Coordinates discharge planning and prepares summaries with patient instructions for transfers.
