Option Care Health, Inc. is seeking an Infusion Nurse II to provide intermediate direct patient care in the home or an alternate infusion suite, ensuring patient safety under a physician’s plan of care.

The role involves coordinating care, administering medications, educating patients and families, documenting visits, and collaborating with the Clinical team to ensure seamless continuity of care. Requires RN licensure, 1+ year nursing experience, and CPR.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates and implements intermediate nursing plan of care treatments.
• Organizes direct patient care and educates patients and families.
• Modifies treatment plans based on patient status and physician orders.
• Prepares clinical documentation in real-time for billing and care continuity.
• Oversees paraprofessionals in the home setting and mentors others as needed.
