Hackensack Meridian Health at Home is seeking an experienced Home Care Infusion RN to deliver professional nursing care in patients' homes, assessing needs, developing and implementing care plans, and evaluating outcomes. This role focuses on one-to-one patient care with a flexible schedule in a home setting.

You will work under policies and regulations, demonstrate infusion competency, coordinate with physicians and the care team, document in the EMR, and meet productivity expectations.

Qualifikationen

• RN licensed in New Jersey with current license.
• Minimum 1 year of RN experience.
• Infusion therapy knowledge or training required.

Aufgaben

• Provide professional nursing care at home with infusion therapy.
• Ensure physician orders followed; document progress toward goals.
• Follow policy with care planning and evaluation.
• Coordinate with interdisciplinary team and document activities.
• Maintain communications with payors and obtain authorizations.
• Complete medication reconciliation and update Plan of Care.
