Job summary

Hackensack Meridian Health At Home is seeking a Home Infusion RN to provide expert nursing care in patients' homes, performing assessments, developing and evaluating care plans, and delivering infusion therapy with a focus on patient-centered, 1:1 care. Responsibilities include documentation, coordination with physicians, and teaching patients and families, with a starting rate of $43.40 per hour.

A NJ RN license and BLS are required, and candidates should have strong communication and computer

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of one (1) year of RN experience.
• Ability/Experience with inserting IVs.
• If not previously trained, will complete a VAD course through HMH and practicum in infusion setting.

Responsibilities

• Provide professional nursing care in patients' homes, assessing needs and implementing care plans.
• Ensure orders and documentation reflect physician directives and discharge goals.
• Document care plans, assessments and interventions accurately in the EMR.
• Coordinate with payors and maintain authorization ranges for visits.
• Teach patients and caregivers as part of the care plan.
• Act as liaison among physicians and health-care team to ensure coordinated care.
