Qualifications

Responsibilities

• Provide professional nursing care through assessment and evaluation of home nursing care needs.
• Demonstrate competency with infusion therapy.
• Ensure compliance with physician orders and documentation.
• Conduct nursing assessments and medication reconciliation.
• Collaborate with physicians on the Plan of Care.
• Implement teaching strategies to meet established goals.
• Act as a liaison among physicians and the healthcare team.
• Complete routine assignments accurately and independently.
• Respond to after-hours messages promptly.
• Maintain awareness of new standards and incorporate them into practice.
