Job summary

Hackensack Meridian Health Home Care Infusion is seeking a Registered Nurse to deliver professional home-based care, assessing, planning, implementing and evaluating patient needs with a focus on infusion therapy. The successful candidate will work in patients' homes delivering 1:1 care and maintaining accurate documentation.

NJ RN license and IV experience are required; strong communication and teamwork are essential.

Qualifications

• RN with required NJ license and at least 1 year of experience.
• Experience with IV placement preferred or willing to complete vascular access training.
• Strong communication and documentation skills.

Responsibilities

• Provide professional nursing care in patients' homes.
• Assess, plan, implement and evaluate home nursing care needs.
• Demonstrate infusion therapy competency and follow policy and procedures.
• Coordinate with physicians, payors and team members for care plans.
• Document assessments, interventions and plans in EMR timely.
