Job summary

Hackensack Meridian Health At Home is seeking a Home Infusion RN to deliver high-quality nursing care in patients' homes across New Jersey. You will assess, plan, implement, and evaluate individualized infusion therapies while collaborating with physicians and an interdisciplinary team.

This role requires RN licensure in New Jersey, 1+ year infusion experience, and strong patient education and documentation skills. A supportive team, flexible scheduling, and comprehensive benefits are provided.

Qualifications

• RN Infusion experience and nursing licensure.
• Ability to insert IVs and manage infusion therapy.
• Strong communication and documentation skills.

Responsibilities

• Provide professional nursing care in patients' homes.
• Assess, implement, and evaluate home infusion plans.
• Document care using EMR and coordinate with physicians.
• Maintain compliance with physician orders and authorization requirements.
• Educate patients and family on infusion procedures and care.
• Be on-call for after-hours visits as needed.
